PAYMENT MADE (Name Tag order will only be processed upon receipt of payment and order form. Do input your child's name upon checking out at the notes.)

Note:
• Please ensure all fields are accurate.
• Vendor is not held responsible for error in spelling and wrong colour selection by customer.
• Average delivery lead time is 4 weeks.
• NameTags be delivered separately directly to your child's class. As such it will not be delivered together with your other items!
• Please refer to 2019 Name Tag Guide if you are uncertain of the name tag colour.
• There will be NO name tag delivery to the school during school holidays.
